Receive, review, and process incoming medical documents.

Index and upload documents into the Electronic Health Record (EHR) system.

Verify patient identifiers and document accuracy before indexing.

Classify documents into appropriate document types.

Ensure completeness and quality of scanned documents.

Perform quality audits on scanned and indexed records.
-
Resolve document mismatches and duplicate records.
-
Maintain document retention and archival procedures.
-
Support document retrieval requests from internal stakeholders.
Release of Information (ROI)
-
Process medical record requests from patients, physicians, attorneys, insurance companies, and government agencies.
-
Verify authorization forms for completeness and HIPAA compliance.
-
Validate requester identity and legal authority.
-
Determine appropriate records for release based on authorization.
-
Redact protected information when required.
-
Release records within regulatory turnaround times.
-
Document all disclosures in accordance with HIPAA accounting requirements.
-
Maintain confidentiality of Protected Health Information (PHI).
-
Handle subpoenas, court orders, and legal requests according to organizational policies.
-
Communicate with requestors regarding incomplete or invalid authorizations.
Compliance & Quality

Ensure compliance with HIPAA, HITECH, CMS, Joint Commission, and organizational policies.

Maintain confidentiality of patient information at all times.
-
Participate in internal quality audits.
-
Identify process improvement opportunities.
-
Report privacy or security incidents immediately.
Customer Service
-
Respond professionally to inquiries regarding medical record requests.
-
Provide status updates to requestors.
-
Coordinate with physicians, clinics, and HIM staff to resolve documentation issues.
Required Qualifications
Experience

1–3 years of experience in:

Health Information Management (HIM)
-
Medical Records
-
Document Management
-
Release of Information (ROI)
-
Healthcare BPO (preferred)
Required Skills

Knowledge of medical terminology

Understanding of HIPAA Privacy Rule

Knowledge of Release of Information regulations

Familiarity with Electronic Health Records (Epic, Cerner, Meditech, Allscripts, eClinicalWorks, etc.)
