The Social Worker will support children with developmental needs and their families by providing guidance, counseling, case management, parent support, and coordination with the multidisciplinary team. The role involves maintaining documentation, conducting follow-ups, and ensuring effective communication between families and therapists.
Key Responsibilities
- Conduct parent interviews and collect case history information.
- Provide emotional support and guidance to parents and caregivers.
- Coordinate and participate in case discussions and multidisciplinary meetings.
- Maintain and update client records, reports, and documentation.
- Conduct follow-up calls with parents regarding therapy progress and concerns.
- Assist families in understanding therapy recommendations and intervention plans.
- Support admission, assessment, and discharge procedures.
- Facilitate parent feedback collection and satisfaction surveys.
- Coordinate awareness programs, workshops, parent training sessions, and community outreach activities.
- Liaise with schools, hospitals, and other agencies when required.
- Prepare social work assessments, reports, and case summaries.
- Ensure confidentiality and ethical handling of client information.
- Assist in organizing events, support groups, and developmental programs.
- Monitor client attendance and communicate with families regarding appointments and schedules.
- Collaborate with Occupational Therapists, Speech-Language Pathologists, Psychologists, Special Educators, and other professionals to ensure holistic care.
Pay: ₹12,000.00 - ₹15,000.00 per month
