We are looking for a detail-oriented and experienced Clinical Quality Assurance Specialist to join our growing team and uphold our commitment to clinical excellence.
Position Summary
The Clinical Quality Assurance (QA) Specialist is responsible for ensuring that all clinical documentation, patient care plans, and agency practices comply with federal, state, and local regulations. This role is pivotal in maintaining our high standards of care through rigorous chart audits, OASIS review, and driving our Quality Assurance Performance Improvement (QAPI) initiatives.
Key Responsibilities
- Documentation & Chart Audits: Review clinical notes, comprehensive assessments, and Plans of Care (485) to ensure accuracy, completeness, and timely submission.
- OASIS & ICD-10 Review: Scrutinize OASIS assessments for accuracy and consistency to ensure optimal patient outcomes and appropriate reimbursement.
- Regulatory Compliance: Ensure all clinical documentation adheres to Medicare Conditions of Participation (CoPs), California Title 22, Medi-Cal, and California Department of Public Health (CDPH) regulations.
- QAPI Program: Actively participate in the Quality Assurance Performance Improvement (QAPI) committee; assist in identifying trends, developing corrective action plans, and monitoring quality indicators.
- Staff Education: Provide constructive feedback and ongoing training to the clinical staff regarding documentation standards, regulatory updates, and best practices.
- Survey Readiness: Assist the leadership team in maintaining continuous readiness for state, federal, and accreditation (e.g., CHAP, Joint Commission) surveys.
- Software Adoption: Champion the use of our Electronic Medical Record (EMR) system, ensuring all clinical staff utilize the tool effectively and correctly.
Qualifications & Clinical Criteria
- Licensure: Active and unencumbered license as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN)
- Experience: Minimum of [1-2] years of clinical experience in a Home Health Care setting.
- QA Experience: Prior experience in Quality Assurance, clinical auditing, or case management is highly preferred.
- Certifications: OASIS Certification (COS-C or HCS-O) and/or Coding Certification (BCHH-C) is a strong plus.
- Knowledge: Deep understanding of California state regulations (Title 22), Medicare guidelines, and home health reimbursement models (PDGM).
- Technical Skills: Proficiency in Home Health EMR software (e.g., [insert EMR name, like Kinnser, Homecare Homebase, or Axxess]).
- Soft Skills: Excellent analytical, communication, and organizational skills with a keen eye for detail.
Pay: ₹36,000.00 - ₹40,000.00 per month
