BACKGROUND
The Hans Foundation (THF), established in 2009, is a Public Charitable Trust dedicated to creating an equitable society and enhancing the quality of life of marginalized and underprivileged communities across India.
THF primarily focuses on improving the health and well-being of remote and underserved populations, with a special emphasis on children, women, and persons with disabilities. In addition, the Foundation works towards holistic development through interventions in the areas of education and livelihood enhancement.
THF's two-fold strategy focuses on:
- Working within Aspirational Districts and regions with a high poverty index across North, North-East, and East India; and
- Addressing the needs of lower-income and migratory populations through targeted urban programs.
Over the past 15 years, THF has impacted more than 57 million beneficiaries through its diverse programs and interventions. The Foundation implements projects directly on the ground while also providing management, monitoring, and technical support to not-for-profit organizations in India funded through THF USA and RIST.

To support the effective implementation of the CMAM program at the field level by ensuring timely identification, referral, follow-up, and management of malnourished children, along with strengthening community engagement and coordination with government systems.
Key Responsibilities

Coordinate and support implementation of CMAM activities at community and block level
-
Ensure active screening and identification of SAM/MAM children through field visits and community platforms

Facilitate referral of severely malnourished children to MTC and ensure proper follow-up after discharge

Support growth monitoring, MUAC measurement, and maintenance of beneficiary records
-
Conduct home visits and counselling sessions for caregivers on IYCF, hygiene, and nutrition practices

Coordinate with Anganwadi Workers (AWWs), ASHAs, ANMs, and health officials for service delivery

Support organization of community-based events (Godh Bharai, Annaprashan, VHSND, etc.)

Ensure timely data collection, reporting, and documentation as per project requirements

Track progress of identified children and ensure regular follow-ups and recovery monitoring

Assist in capacity building and on-field handholding of frontline workers

Maintain coordination with Community Development Specialist for smooth operations
Required Qualification & Experience :
-
Graduate
-
Minimum 2 years of experience in community-based programs, preferably in health/nutrition or CMAM
-
Experience of working with government systems (ICDS/Health Department) preferred
Skills & Competencies :
-
Good understanding of malnutrition (SAM/MAM) and community health practices

Strong communication and counselling skills in Hindi (local language preferred)

Ability to work closely with communities and frontline workers

Basic knowledge of MS Office and data reporting

Strong coordination, problem-solving, and field management skills

Commitment to public health and community development
Other Requirements :

Willingness to travel extensively in rural areas

Field Visits: Minimum 20 days per month in project villages
