Job Summary
The MRD Executive is responsible for maintaining, organizing, securing, and retrieving patient medical records in accordance with hospital policies, statutory requirements, and accreditation standards (NABH/NABL, where applicable). The role ensures the confidentiality, accuracy, and timely availability of medical records while supporting clinical, legal, insurance, and administrative requirements.
Key Responsibilities
Medical Records Management
- Maintain complete and accurate inpatient and outpatient medical records.
- Receive, verify, arrange, and file patient records after discharge.
- Ensure all medical records are completed by consultants before final filing.
- Retrieve and provide medical records to authorized departments as per hospital policy.
- Maintain both physical and electronic medical records.
Documentation & Compliance
- Check medical records for completeness, including:
- Admission forms
- Consent forms
- Clinical notes
- Investigation reports
- Nursing records
- Medication charts
- Discharge summaries
- Coordinate with doctors and nursing staff to complete deficient records.
- Ensure compliance with NABH standards, hospital policies, and legal requirements.
- Maintain confidentiality of patient information at all times.
Record Storage & Retrieval
- File medical records systematically for easy retrieval.
- Maintain record movement registers or electronic tracking systems.
- Ensure proper storage and preservation of records.
- Manage archival and destruction of records as per retention policy.
Reporting & MIS
- Prepare daily, weekly, and monthly MRD reports.
- Maintain records of discharged files, pending files, and incomplete records.
- Generate statistics required by management and quality departments.
Coordination
- Coordinate with Billing, Nursing, Consultants, Insurance (TPA), Quality, and Legal departments.
- Provide records for audits, insurance claims, medico-legal cases, and accreditation assessments.
- Assist internal and external auditors during record verification.
Digital Record Management
- Update patient records in the Hospital Information Management System (HIMS).
- Scan and upload required documents into the electronic medical record system.
- Ensure data accuracy and timely completion of digital documentation.
Key Performance Indicators (KPIs)
- Percentage of complete discharge files within defined TAT.
- Retrieval time for requested medical records.
- Number of pending incomplete records.
- Accuracy of medical record documentation.
- Compliance with NABH and hospital documentation standards.
- Timely submission of MRD reports.
- Zero breaches of patient confidentiality.
Required Skills
- Knowledge of medical terminology.
- Familiarity with hospital documentation standards.

- Strong attention to detail.
- Proficiency in MS Office and Hospital Information Systems (HIMS).
Pay: ₹30,000.00 - ₹50,000.00 per month
