Job Summary
The Medical Records Department (MRD) Executive is responsible for maintaining accurate, complete, and confidential patient medical records while ensuring compliance with hospital policies, statutory requirements, and quality standards. The role includes medical record documentation, coding, filing, statistical reporting, quantitative analysis, and coordination with various departments to ensure efficient medical records management.
Qualification
Mandatory:
- Diploma in Medical Record Technology (MRD) / Diploma in Medical Record Science.
- Bachelor's degree in any discipline is preferred.
Experience
- 1–5 years of experience in a Hospital Medical Records Department.
- Experience in NABH-accredited hospitals will be an added advantage.
Key ResponsibilitiesMedical Records Management
- Maintain, organize, and securely store inpatient and outpatient medical records.
- Assemble patient files accurately and ensure all required documents are available before filing.
- Maintain proper filing and retrieval systems for current and archived medical records.
- Sort, file, and preserve old medical records as per hospital retention policies.
- Ensure confidentiality and security of all patient records in accordance with hospital policies and applicable regulations.
Medical Coding & Documentation
- Perform accurate disease and procedure coding for discharged patients using ICD-10 and ICPM coding standards.
- Ensure proper documentation and coding accuracy for billing, reporting, and clinical purposes.
- Coordinate with clinicians to clarify incomplete or inaccurate documentation when required.
Quantitative Analysis & Quality Review
- Conduct quantitative analysis of medical records to verify completeness and accuracy.
- Review discharge summaries, operation notes, consent forms, investigation reports, and other clinical documentation for completeness.
- Identify missing documentation and coordinate with concerned departments for timely completion.
- Support technical evaluation and continuous improvement of medical record processes.
Statistical Reports & Compliance
- Prepare and submit monthly hospital statistical reports.
- Compile and forward statutory reports, including:
- Hospital Monthly Reports
- Communicable Disease Reports
- Medical Termination of Pregnancy (MTP) Reports
- Other regulatory reports as required
- Maintain the confidentiality and accuracy of all statistical returns.
Quality Management
- Coordinate MRD activities related to the Quality Management System (QMS).
- Support NABH and other accreditation requirements through proper documentation and record management.
- Participate in departmental audits and quality improvement initiatives.
Coordination & Administration
- Coordinate medical record activities with other departments and group hospitals whenever required.
- Assist clinicians, nursing staff, and other departments in retrieving medical records promptly.
- Ensure timely completion of departmental documentation and reports.
