Wanted Clinical Pharmacist with a minimum of 1 year experience
Key Responsibilities:
Review medication orders and ensure appropriate drug therapy.
Monitor patient medication safety and efficacy.
Provide drug information and counseling to healthcare professionals and patients.
Identify and prevent medication errors and adverse drug reactions.
Participate in ward rounds and collaborate with the multidisciplinary healthcare team.
Maintain accurate clinical documentation and ensure compliance with hospital protocols.
