1. Clinical Care
- Provide direct nursing care to patients in accordance with established standards, CCF protocols, policies, and care plans, ensuring full accountability for patient care within the assigned area.
- Perform comprehensive nursing assessments (admission initial assessment and daily assessment) and monitor changes in patients’ conditions.
- Ensure all special assignments are completed accurately and on time.
- Administer medications and treatments safely and accurately, in accordance with physician orders and facility policies.
- Ensure accurate and timely documentation in patient records.
- Collaborate with the interdisciplinary care team (physicians, therapists, dietitians, etc.) to develop and revise care plans.
- Maintain accurate and timely documentation in the inpatient file/ electronic health record (EHR).
- Provide palliative and end-of-life care consistent with patient and family wishes.
- Maintain strict confidentiality of patient and staff-related information.
2. Coordination & Communication
- Ensure continuity of care through clear and concise verbal or written communication.
- Facilitate smooth transitions between levels of care (e.g., hospital to rehab, rehab to home).
- Participate in shift handovers and interdisciplinary case discussions. Accurately and thoroughly endorse patients and assigned tasks to the next shift.
- Promptly report any changes or deviations in the patient’s condition to the appropriate personnel as required.
- Accurately and thoroughly document the care provided in a clear and concise way, adhering to established protocols.
- Communicate effectively with patients, families, and the healthcare team regarding care plans and progress.
- Address patient and family queries and concerns in a professional manner.
- Manage smooth patient admission, transfer, and discharge processes.
- Coordinate with multidisciplinary teams for comprehensive patient care.
3. Safety & Quality
- Adhere to infection control, medication administration, and safety policies & report any deviations/ safety concerns.
- Initiates emergency resuscitative measures in accordance with BLS and ACLS protocols and serves as the first responder when required.
- Participates actively in quality improvement and risk management activities.
- Report and document incidents, near misses, and changes in patient status promptly.
- Ensure compliance with NABH and other accreditation standards.
4. Education & Professional Development
- Provide health education to patients, families, and caregivers.
- Stay updated on best practices in critical care nursing, participate in ongoing education to remain current with best practices in long term acute care, geriatric and continuum care nursing.
