The Staff Nurse is responsible for providing safe, compassionate, and evidence-based nursing care to patients. The Staff Nurse shall assist doctors during diagnosis and treatment, administer medications, maintain accurate documentation, ensure infection prevention, and promote patient safety in accordance with hospital policies and standard nursing practices.
1. GENERAL RESPONSIBILITIES
- Report to duty on time in complete uniform with ID card.
- Receive proper shift handover and hand over all patient details at the end of duty.
- Deliver safe, ethical, and patient-centered nursing care.
- Follow hospital policies, nursing protocols, and infection control guidelines.
- Maintain patient privacy and confidentiality.
- Treat patients, relatives, and colleagues with dignity and respect.
- Inform the Nursing Incharge/ICU Incharge immediately regarding any emergency or abnormal patient condition.
- Maintain cleanliness and discipline in the assigned ward/unit.
2. PATIENT CARE RESPONSIBILITIES
- Assess patients on admission and during each shift.
- Monitor and record vital signs accurately.
- Administer medications as prescribed by the doctor following the Five Rights (Right Patient, Right Drug, Right Dose, Right Route, Right Time).
- Start and maintain IV fluids and intravenous medications.
- Administer injections safely using aseptic techniques.
- Perform wound dressing and assist in minor procedures.
- Insert and care for urinary catheters as per doctor's orders.
- Provide nasogastric (Ryle's tube) feeding and care.
- Administer oxygen therapy as prescribed.
- Monitor blood glucose levels and administer insulin as ordered.
- Collect blood, urine, sputum, and other laboratory samples.
- Monitor intake and output accurately.
- Observe and report changes in the patient's condition immediately.
- Assist patients with personal hygiene, feeding, mobility, and elimination.
- Prevent pressure injuries by regular repositioning of bedridden patients.
- Educate patients and relatives regarding treatment, medications, diet, and discharge instructions.
3. EMERGENCY RESPONSIBILITIES
- Recognize early signs of patient deterioration.
- Respond immediately to medical emergencies.
- Assist in CPR and Basic Life Support (BLS)/Advanced Cardiac Life Support (ACLS).
- Prepare emergency drugs and equipment.
- Ensure crash cart readiness during every shift.
- Assist doctors during emergency procedures such as intubation, central line insertion, and defibrillation.
- Arrange immediate transfer of critically ill patients when required.
4. MEDICATION MANAGEMENT
- Verify doctor's orders before medication administration.
- Check patient identity using at least two identifiers.
- Check medication expiry dates before administration.
- Monitor patients for adverse drug reactions.
- Record all medications immediately after administration.
- Maintain narcotic drug register where applicable.
- Report medication errors immediately.
5. DOCUMENTATION RESPONSIBILITIES
Maintain complete and accurate records, including:
- Nursing Assessment Sheet
- Nursing Progress Notes
- Vital Signs Chart
- Medication Administration Record (MAR)
- Intake and Output Chart
- IV Fluid Chart
- Dressing Register
- Procedure Notes
- Admission and Discharge Records
- Blood Transfusion Records
- Incident Report (if applicable)
- Death Record Documentation (if applicable)
6. INFECTION PREVENTION & CONTROL
- Perform hand hygiene before and after every patient contact.
- Use appropriate Personal Protective Equipment (PPE).
- Follow standard and transmission-based precautions.
- Maintain sterile techniques during procedures.
- Segregate biomedical waste correctly using color-coded bins.
- Dispose of needles in puncture-proof sharps containers.
- Clean and disinfect patient care equipment after use.
- Report needle-stick injuries immediately.
- Follow isolation protocols for infectious patients.
7. EQUIPMENT MANAGEMENT
- Check the functioning of all equipment before use.
- Ensure availability of:
- BP apparatus
- Pulse oximeter
- Thermometer
- Glucometer
- Suction apparatus
- Oxygen cylinders/flow meters
- Nebulizer
- ECG machine
- Infusion pumps and syringe pumps (where available)
- Report equipment malfunction immediately.
- Maintain equipment cleanliness and safety.
8. PATIENT SAFETY
- Verify patient identity before medications, procedures, and blood transfusions.
- Prevent patient falls using fall prevention protocols.
- Raise side rails for vulnerable patients.
- Ensure safe transfer of patients between departments.
- Monitor patients receiving blood transfusions.
- Report adverse events immediately.
- Ensure informed consent is available before procedures.
9. WARD MANAGEMENT
- Maintain ward cleanliness and organization.
- Ensure adequate stock of medicines and consumables.
- Check emergency trolley/crash cart daily.
- Monitor oxygen supply.
- Maintain linen and inventory.
- Ensure beds are clean and ready for new admissions.
- Coordinate with housekeeping for cleanliness.
10. COMMUNICATION RESPONSIBILITIES
- Communicate patient status clearly during shift handovers.
- Inform doctors promptly about abnormal findings.
- Coordinate with Laboratory, Pharmacy, Radiology, Operation Theatre, ICU, and Billing departments.
- Explain nursing procedures to patients and relatives.
- Maintain courteous and professional communication at all times.
11. DISCHARGE RESPONSIBILITIES
- Verify doctor's discharge order.
- Educate patients regarding medications, diet, wound care, activity, and follow-up.
- Remove IV cannula and other devices as instructed.
- Complete discharge documentation.
- Ensure all hospital property is returned.
- Prepare the bed for the next patient.

- Be respectful, compassionate, and professional.
- Refrain from accepting gifts or money from patients or relatives.
- Participate in continuing nursing education and training.
14. REPORTING RESPONSIBILITIES
Report immediately to the Nursing Incharge/Medical Officer:
- Patient deterioration
- Cardiac arrest

- Falls or injuries
- Needle-stick injuries
