Bpharma/Dpharma
- Clinical Care and Monitoring: Reviewing patient medication charts to ensure safe prescribing, checking for potential allergies or drug interactions, and providing pharmaceutical care to inpatients.
- Dispensing and Safety: Preparing, storing, and dispensing medications, including specialty items like intravenous (IV) solutions and chemotherapy, ensuring accuracy and quality.
- Patient Counseling: Advising patients on medication management, side effects, and adherence, particularly at discharge, to ensure safe transitions to home care.
- Drug Procurement and Management: Overseeing the hospital’s drug supply chain, managing inventory, and ensuring the formulary system remains effective and cost-efficient.
