- Conduct comprehensive assessments of older adults to identify physical, psychological, social, and functional health needs.
- Develop and implement individualized care plans tailored to the client’s medical conditions and personal preferences.
- Administer medications safely and monitor for therapeutic effects, adverse reactions, and potential drug interactions.
- Provide skilled nursing care, including wound management, vital sign monitoring, and chronic disease management.
- Promote independence by supporting activities of daily living while preserving the client’s dignity and autonomy.
- Educate patients and family members on disease management, medication adherence, nutrition, and healthy lifestyle practices.
- Monitor cognitive and emotional well-being, identifying early signs of depression, dementia, or other age-related concerns.
- Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity and quality of care.
- Implement fall-prevention and safety measures to create a secure and supportive home environment.
- Advocate for the rights, comfort, and overall well-being of older adults, ensuring compassionate and person-centered care.
Pay: ₹18,000.00 - ₹25,000.00 per month
