Job Summary
We are seeking a M.sc qualified nurse with strong English communication skills to support our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) operations.
The role involves monitoring patient readings, responding to critical alerts, coordinating with care managers, uploading care plans, and preparing submissions in accordance with compliance requirements.
Key Responsibilities
- Monitor patients enrolled in RPM and CCM programs.
- Review patient readings and health data available on the monitoring platform.
- Identify critical alerts, abnormal readings, and concerning changes in patient data.
- Call patients promptly when critical alerts are received.
- Collect relevant information from patients during alert-related calls.
- Document patient calls, readings, alerts, and actions taken.
- Escalate critical alerts to the appropriate care manager according to established protocols.
- Coordinate with care managers regarding patient alerts, follow-ups, care plans, and required actions.
- Upload patient care plans into the designated platform or system.
- Review care plans and supporting documents for completeness before uploading.
- Maintain accurate and updated patient records.
- Prepare RPM and CCM submissions according to applicable compliance requirements.
- Verify that all required documentation is complete before submission.
- Ensure submissions are prepared accurately and within the required timelines.
- Maintain proper documentation for compliance and audit purposes.
- Follow established patient confidentiality, documentation, and data security procedures.
Required Qualifications
- M.sc degree, or equivalent qualification in Nursing.
- Valid nursing registration or licence.
- Good spoken and written English communication skills.
- Ability to communicate clearly and professionally with patients and care managers.
- Basic knowledge of patient vital signs and clinical readings.
- Ability to identify critical or abnormal patient readings.
- Good computer skills and ability to work with online healthcare platforms.
- Strong documentation and record-management skills.
- Ability to work during U.S. Central Time hours.
Preferred Qualifications
- Previous experience in Remote Patient Monitoring or Chronic Care Management.
- Experience monitoring patient readings such as blood pressure, blood glucose, oxygen saturation, weight, and heart rate.
- Experience working with care managers or remote healthcare teams.
- Familiarity with healthcare documentation and compliance requirements.
- Experience using RPM, CCM, Electronic Health Record, or care-management platforms.

Pay: ₹50,000.00 - ₹60,000.00 per month
