Patient Counseling & Support:
· Offer emotional support and counseling to patients dealing with serious illness, trauma, or chronic conditions.
· Assist families in coping with diagnoses, treatment decisions, grief, and end-of-life care.
Discharge Planning:
· Coordinate with doctors, nurses, and families to plan safe and effective discharges.
· Help arrange follow-up care, home care, or transfer to rehabilitation or hospice facilities.
Financial & Social Assistance:
· Identify patients in financial distress and assist in applying for government schemes, NGO support, or charitable trust funds.
· Prepare and maintain documentation for availing financial aid.
Patient Advocacy & Liaison:
· Act as a bridge between patients/families and the medical team, ensuring clear communication and addressing concerns.
· Advocate for patients’ rights, dignity, and privacy.
Community Resource Coordination:
· Connect patients to external support services such as counseling centers, rehabilitation facilities, blood banks, and palliative care services.
· Collaborate with NGOs and community-based organizations for patient welfare.
Documentation & Reporting:
· Maintain detailed case records, social investigation reports, and progress notes.
· Prepare reports for audits, CSR programs, and patient assistance schemes.
Health Awareness & Education:
· Organize and participate in health education sessions, camps, and awareness drives.
· Educate patients and families on disease management, hygiene, nutrition, and available support systems.
