Job Summary:
We are seeking a compassionate, organized, and experienced Freelance Care Manager to support our elder care clients in maintaining their independence, dignity, and quality of life. As a freelance professional, you will coordinate care plans, liaise with healthcare providers, and ensure that each client receives personalized, holistic support tailored to their needs.
Key Responsibilities:
· Conduct comprehensive assessments of clients’ physical, emotional, social, and environmental needs
· Develop and implement individualized care plans in collaboration with clients, families, and healthcare providers
· Monitor and evaluate care plans regularly, adjusting as needed
· Coordinate services such as home health aides, medical appointments, transportation, and community resources
· Advocate for clients’ rights and well-being
· Maintain accurate documentation and reports
· Provide emotional support and guidance to clients and families
· Respond promptly to emergencies or changes in client condition
Pay: ₹15,000.00 - ₹22,000.00 per month
