We are seeking a dedicated and patient-focused Clinical Pharmacist to join our healthcare team. The ideal candidate will work closely with doctors, nurses, and other healthcare professionals to ensure the safe, effective, and rational use of medications while supporting NABH quality standards and patient safety initiatives.
Key Responsibilities
- Review medication orders for accuracy, appropriateness, and drug interactions.
- Provide clinical pharmacy services, including medication counseling and patient education.
- Participate in ward rounds and collaborate with the healthcare team for optimal patient care.
- Monitor adverse drug reactions (ADR) and medication errors and report them appropriately.
- Ensure compliance with NABH standards, hospital policies, and medication safety protocols.
- Support antimicrobial stewardship and rational antibiotic use.
- Maintain accurate clinical documentation and pharmacy records.
- Educate healthcare staff on medication safety and best practices.
Requirements
- Pharm.D / M.Pharm (Pharmacy Practice) or B.Pharm with relevant clinical experience.
- Minimum 1 year of experience as a Clinical Pharmacist in a hospital.
- Good knowledge of pharmacotherapy, medication safety, and NABH standards.
- Strong communication, analytical, and teamwork skills.
- Proficiency in hospital information systems and MS Office is an added advantage.
