Job Summary
We are looking for a compassionate, detail-oriented, and proactive Healthcare Case Manager to coordinate patient care and international medical consultations during the evening/night shift. The primary purpose of this shift is to facilitate seamless communication and coordination with international specialists, hospitals, and healthcare providers across multiple time zones.
The ideal candidate will serve as the primary point of contact for patients, ensuring timely scheduling of consultations, continuity of care, follow-ups, and exceptional patient support throughout their healthcare journey.
Additional Responsibilities – International Consultation Coordination
- Coordinate virtual consultations and second opinions with international specialists and healthcare providers across different time zones.
- Schedule appointments by aligning the availability of patients, physicians, and international medical teams.
- Prepare and share relevant medical records, diagnostic reports, imaging, and clinical summaries with consulting specialists prior to consultations.
- Join and coordinate international consultation calls when required, ensuring smooth communication between patients, physicians, and caregivers.
- Document consultation outcomes, recommendations, and treatment plans, and coordinate subsequent follow-ups with the treating medical team.
- Liaise with international hospitals, specialist clinics, diagnostic centres, and healthcare partners to facilitate continuity of care.
- Coordinate urgent international consultations for critical or time-sensitive cases outside regular Indian working hours.
- Ensure timely communication with patients and families regarding consultation schedules, outcomes, and next steps.
- Maintain accurate documentation of all international communications and consultation records within the CRM/EMR system.
- Collaborate with internal physicians and care teams to implement recommendations received from international consultants.
Pay: ₹35,000.00 - ₹60,000.00 per month
