Key Responsibilities:
- Medication Management:
- Review and verify medication orders
- Dispense and administer medications
- Monitor patient medication profiles
- Patient Care:
- Counsel patients on medication use
- Provide medication information to healthcare teams
- Participate in patient care rounds
- Collaboration:
- Work with healthcare teams to optimize medication therapy
- Participate in medication-related committees
- Quality Assurance:
- Ensure compliance with medication policies and procedures
- Monitor medication safety and efficacy
- Report medication errors and adverse reactions
