JOB DESCRIPTION (JD): OBSTETRIC REGISTRAR (24x7 COVERAGE)
1. POSITION DETAILS
- Designation: Obstetric Registrar
- Qualification: MBBS with DGO / DNB / MS (OBG)
- Experience: 0–2 years
- Reporting To: Consultant Obstetrician & Medical Director.
- Location: Aastrika Midwifery Centre – Bangalore.
2. ROLE SUMMARY
The Obstetric Registrar functions as the primary in-house clinical responder providing 24/7 obstetric and gynecological care within a midwifery-led model.
The Registrar will:
- Work under the supervision of the Consultant Obstetrician
- Be responsible for early identification of complications, initiation of emergency management, and time-bound escalation
- Ensure safe, evidence-based, protocol-driven maternal and newborn care
- Coordinate with anesthesia, OT, ICU, and pediatric teams of the hospital and partners.
3. CLINICAL GOVERNANCE & CHAIN OF COMMAND
3.1 Supervision Structure
- Registrar works under direct/indirect supervision of Consultant Obstetrician
- Consultants are final decision-makers in all high-risk and operative cases
3.2 Chain of Command
- Obstetric Registrar (In-house 24x7)
- Consultant Obstetrician (On-call / On-site)
- Medical Director / Clinical Head
3.3 Escalation Responsibility : The Registrar must:
- Recognize clinical deterioration early
- Initiate immediate management
- Escalate without delay as per defined triggers
- Document time, reason, and response to escalation
4. TIME-BOUND ESCALATION TRIGGERS (MANDATORY)
The Registrar MUST inform the Consultant immediately in all high risk situations in Maternal & Fetal Conditions
Procedural / System Triggers
- Decision for cesarean section
- Need for operative vaginal delivery
- Requirement for ICU transfer
- Any deviation from standard protocol
5. CLINICAL RESPONSIBILITIES
5.1 Antenatal & OPD Care
- Conduct antenatal consultations and risk stratification
- Identify and monitor high-risk pregnancies
- Order and interpret investigations
- Provide counselling on:
- Nutrition
- Birth preparedness
- High-risk conditions
5.2 Labour & Delivery Care
- Promote physiological labour in collaboration with midwives
- Monitor labour using partograph and CTG
- Conduct normal vaginal deliveries (as per competence)
- Assist in:
- Operative vaginal delivery
- Cesarean section
- Implement labour protocols and safety bundles
- Recognize complications early and act promptly
5.3 Emergency Response & Stabilization
The Registrar is responsible for initial management and stabilization of all mothers/babies till there is a proper hand over to the appropriate consultant.
5.4 Postnatal & Newborn Care
- Monitor maternal recovery
- Identify postpartum complications
- Provide breastfeeding support
Neonatal Responsibility
- Perform neonatal resuscitation as per NRP protocol
- Stabilize newborn until pediatrician takeover
- Ensure timely NICU referral
5.5 Gynecological Responsibilities
- Perform minor procedures
- Assist in gynecological surgeries
6. INTERDEPARTMENTAL COORDINATION
The Registrar shall:
- Coordinate with Anesthesia Team for emergency OT activation
- Liaise with ICU Team for critically ill patients
- Work closely with Pediatricians / NICU Team
- Ensure:
- Timely referrals
- Safe transfers
- Proper handover documentation
7. MIDWIFERY MODEL SUPPORT
- Collaborate with midwives respectfully
- Promote minimal intervention practices
- Support informed maternal choice
- Ensure safe escalation when required
8. MEDICO-LEGAL RESPONSIBILITIES
The Registrar must demonstrate strong medico-legal awareness and documentation skills.
Mandatory Responsibilities

- Partograph
- CTG records
- Case sheets with time-stamped entries
- Document:
- All clinical decisions
- Escalations
- Emergency actions
- Ensure compliance with:

- Legal and ethical standards
9. PROTOCOL ADHERENCE & QUALITY
The Registrar is responsible for:
- Strict adherence to hospital SOPs and clinical guidelines
- Implementation of:
- PPH bundle
- Eclampsia management bundle
- Sepsis protocols
- Ensuring bundle compliance in all applicable cases
10. AUDIT & QUALITY IMPROVEMENT
- Participate in:
- Maternal morbidity and mortality meetings
- Clinical audits
- Case reviews
- Report:
- Adverse events
- Near misses
- Contribute to continuous quality improvement
11. COMMUNICATION & HANDOVER
- Maintain clear communication with:
- Patients and families
- Clinical teams
- Handle sensitive situations empathetically
Handover Responsibility
- Ensure structured shift handover
- Maintain updated patient status and risk categorization
12. INFECTION CONTROL & PATIENT SAFETY
- Adhere to infection control protocols
- Follow sterilization standards
- Participate in safety and infection control programs
13. EDUCATION & TRAINING
- Participate in academic activities
- Support training of nurses and midwives
- Engage in continuous medical education
14. SCOPE OF PRACTICE & LEGAL PROTECTION
- Work within defined competency limits
- All high-risk decisions must be:
- Escalated
- Documented
- Covered under hospital indemnity policy

16. REMUNERATION: Competitive and commensurate with experience
17.WORKING HOURS & LEAVES:
The appointee is expected to be available 24x7 and will cover night care for all mothers. The candidate is expected to work a minimum of 48.5 hours per week and must be available for care till the handover. The eligible leaves and week offs will be managed to ensure adequate coverage and care for the delivering mother.
