My Home Health (MHH) is looking for a compassionate, organised, and proactive Care Manager to coordinate patient care while supporting business growth. If you enjoy working with elderly patients and building relationships with healthcare professionals, we'd love to hear from you.
Key Responsibilities
- Coordinate care for elderly and home healthcare patients.
- Maintain patient records and follow-up documentation.
- Liaise with doctors, nurses, physiotherapists, hospitals, and medical equipment vendors.
- Handle patient enquiries and convert leads into active clients.
- Build referral networks with doctors, clinics, hospitals, and other healthcare partners.
- Coordinate caregiver deployment and ensure high-quality service.
- Prepare basic reports and maintain patient databases.
Desired Profile
- GNM/ANM/B.Sc. Nursing preferred (or any graduate with relevant experience).
- 1–5 years' experience in healthcare, hospital, home care, or patient coordination.
- Excellent communication skills in Bengali, Hindi, and English.
- Good computer skills (MS Office/Excel).
- Strong organisational, coordination, and relationship-building abilities.
