- Patient Assessment: Check and record vital signs, including blood pressure, heart rate, temperature, blood sugar, and oxygen levels, at every visit. [4, 7]
- Medication Administration: Administer prescribed oral medications, injections, insulin, and intravenous (IV) therapies safely and on schedule. [1, 7]
- Wound & Post-Surgical Care: Clean wounds, dress incisions, inspect stitches, and monitor closely for early signs of infection. [1, 7]
- Specialized Device Management: Manage and maintain feeding tubes, urinary catheters, stoma bags, oxygen therapy, or ventilator equipment. [4, 6]
- Emergency Handling: Recognize critical changes in patient status, deliver first aid, and execute emergency protocols calmly.
Case Management & Coordination
- Care Plan Execution: Follow and implement the physician's prescribed medical plan while suggesting adjustments based on real-time recovery.
- Documentation: Maintain accurate, up-to-date physical or electronic health records detailing patient progress, treatments administered, and any vital changes.
- Team Supervision: Guide and oversee home health aides or licensed practical nurses (LPNs) assigned to the patient's care team.
Patient & Family Support
- Health Education: Teach family members and primary caregivers how to properly manage medications, use equipment, and maintain proper hygiene.
- Daily Living Assistance: Provide or assist with personal care tasks, including bathing, grooming, diaper changes, and mobility assistance.
- Safety Assessments: Evaluate the home environment for potential hazards, such as fall risks, to ensure patient safety.
Job Requirements & QualificationsEducation & Licensing
- Degree: Associate’s Degree in Nursing (ADN) or a Bachelor of Science in Nursing (BSN).
- License: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Certification: Passing score on the NCLEX exam and active CPR/BLS certification.
