- Should be flexible to do both Transition care and Home care services.
- Monitoring Pulse, Measuring Blood Pressure, Assessing Respiration Rate, Checking Temperature.
- Assist patients and caregivers with educational needs, problem resolution.
- Administering sponge baths to patients, Accompanying the patients for walk, Repositioning, attending diaper changes and the patient’s toileting needs.
- Ensuring timely administration of prescribed medication.
- Develops patient care plans.
- Assists patients and caregivers with educational needs , problem resolution.
- Providing ongoing evaluation of the quality and effectiveness of the educational activities.
- Complete other tasks as and when assigned.
- Follow company protocols.
- Observe patients and report any changes in patient conditions to the nursing staff or doctor on call.
- Traveling to patients’ homes and managing their care plans according to physicians’ instructions.
- Inspecting wounds, changing dressings, and handling personal grooming and hygiene.
