- Assess the physical, mental, and functional health status of adult and elderly patients at home.
- Develop and implement individualized care plans based on patient needs and medical history.
- Monitor and manage chronic diseases such as diabetes, hypertension, heart disease, and COPD.
- Prescribe and administer medications as authorized and monitor for side effects or adverse reactions.
- Perform comprehensive health assessments and order or interpret diagnostic tests when appropriate.
- Provide wound care, catheter care, and other advanced nursing procedures in the home setting.
- Educate patients and family members about disease management, medications, nutrition, and healthy lifestyle practices.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated care.
- Recognize changes in the patient's condition and initiate timely interventions or referrals when necessary.
- Maintain accurate clinical documentation and ensure high-quality, safe, and patient-centered home healthcare services.
