Job Summary
The Medical Social Worker is responsible for providing psychosocial support, patient counseling, community outreach, and care coordination within the Primary Health Centre. The role focuses on identifying social and economic barriers to healthcare, connecting patients with government and community welfare services, and improving access to quality healthcare through patient education and advocacy.
Key Responsibilities
- Conduct psychosocial assessments to identify patients' social, emotional, and financial needs.
- Provide counseling to patients and their families regarding illness, treatment, rehabilitation, and available support services.
- Assist patients in accessing government health schemes, insurance programs, and social welfare benefits.
- Coordinate referrals to hospitals, specialists, NGOs, rehabilitation centers, and community support services.
- Organize and participate in community outreach programs, health awareness campaigns, and health education activities.
- Support maternal and child health, elderly care, mental health, palliative care, and chronic disease management initiatives.
- Identify vulnerable individuals and families requiring additional social support and follow-up.
- Collaborate with doctors, nurses, ASHAs, ANMs, and other healthcare professionals to ensure holistic patient care.
- Maintain accurate case records, patient documentation, referral registers, and monthly reports.
- Ensure confidentiality and ethical handling of patient information.
Qualifications
- MSW (Medical & Psychiatric Social Work preferred) or BSW with relevant healthcare experience.
- 1–3 years of experience in a hospital, community health program, NGO, or Primary Health Centre.
- Knowledge of public health programs and government healthcare schemes.
- Proficiency in Microsoft Office and documentation practices.

Salary
As per organizational policy and candidate experience.
Pay: ₹15,000.00 - ₹22,000.00 per month
