Key Responsibilities
Clinical Responsibilities
- Conduct skilled nursing visits at patients' homes as per assigned schedule.
- Perform patient assessments and evaluate clinical conditions.
- Develop individualized care plans and care schedules based on assessment findings.
- Maintain accurate medical records and patient documentation.
- Update all patient-related activities in the CET Application.
- Ensure quality and continuity of nursing care for all assigned patients.
Supervision & Quality Monitoring
- Conduct caregiver and patient care supervisions according to the approved schedule.
- Prepare supervision reports immediately after each visit.
- Submit supervision reports to the Nursing Supervisor on time.
- Monitor compliance with care plans and quality standards.
Coordination & Communication
- Coordinate with Customer Care, Logistics, and Finance departments to resolve issues related to Skilled Nursing Services.
- Make pre-visit and post-visit calls to patients/families and maintain records.
- Attend daily operational meetings and provide updates on patient care status.
- Escalate clinical concerns and operational issues to the Nursing Supervisor promptly.
Training & Development
- Conduct training sessions for caregivers and healthcare staff through the CET System as per the training schedule.
- Support skill development initiatives and competency assessments.
- Participate in continuous learning and professional development activities.
Reporting & Documentation
- Maintain a Daily Activity Log.
- Submit periodic performance and operational reports to the Nursing Supervisor.
- Ensure all clinical and supervision records are updated accurately and timely.
- Maintain confidentiality and compliance with healthcare documentation standards.
Continuous Improvement
- Participate in monthly review meetings.
- Propose at least one improvement initiative every month to enhance operational efficiency, patient care quality, or skilled nursing procedures.
- Support implementation of approved process improvement projects.
Requirements
Qualification
- B.Sc Nursing / GNM Requirements
- Valid Indian Nursing Council Registration Certificate
- Valid Driving License
- Minimum 1 year of clinical nursing experience
