- Assess the patient’s health condition at home during every visit or shift and monitor vital signs including BP, pulse, temperature, respiratory rate, and oxygen saturation.
- There will be 12-hour and 24-hour shifts, subject to operational requirements.
- Observe any changes in symptoms, identify deterioration, breathing difficulty, unconsciousness, or other unusual signs, and report immediately to the nursing manager, consultant, or doctor.
- Evaluate mobility, pain level, and daily functioning, and assist with bed bath, diaper change, dress change, hair care, repositioning, exercises, and other personal care needs.
- Administer prescribed treatments including oral medicines, injections, IV fluids, wound dressing, pressure sore care, and post-operative or chronic illness nursing support as per the care plan.
- Provide specialized care such as colostomy bag cleaning, urinary catheter care, Ryle’s tube feeding and care, tracheostomy care and dressing, and PEG tube feeding and care.
- Follow doctors’ instructions and individualized nursing care plans, customize care according to the patient’s home environment, and ensure continuity of care in every visit or shift.
- Maintain cleanliness during all procedures, follow infection control practices including hand hygiene, PPE, aseptic technique, and ensure patient safety through fall prevention and bed safety measures.
- Track the patient’s recovery progress, maintain nursing notes and daily records accurately, and arrange immediate hospital transfer whenever the patient’s condition requires urgent medical attention.
