Key Responsibilities
- Participate in ward rounds and multidisciplinary patient care discussions.
- Review medication orders for appropriateness, dosage, interactions, and contraindications.
- Monitor drug therapy and recommend medication adjustments when required.
- Provide drug information and medication counseling to patients and healthcare professionals.
- Conduct medication reconciliation and therapeutic drug monitoring.
- Monitor and report Adverse Drug Reactions (ADR) and medication errors.
- Support antimicrobial stewardship and rational use of medicines.
- Maintain clinical pharmacy documentation and intervention records.
- Assist in developing hospital medication-use policies and protocols.
- Participate in clinical audits, research activities, and pharmacovigilance programs.
- Guide and supervise Pharm.D interns and pharmacy students during clinical posting
Eligibility Criteria
- Pharm.D / M.Pharm (Pharmacy Practice/Clinical Pharmacy) from a recognized institution.
- Valid registration with the State Pharmacy Council.
- 1–3 years of experience in a hospital or clinical pharmacy setting preferred.
- Freshers with Pharm.D and strong clinical knowledge may also apply.
- Knowledge of pharmacology, therapeutics, drug interactions, and patient counseling.
- Good communication, analytical, and documentation skills.
