Key Responsibilities:
Coordinate the care of transplant patients before, during, and after transplantation.
Assess patients and monitor their physical and emotional condition.
Educate patients and families about the transplant process, medications, and lifestyle changes.
Prepare patients for transplant surgery and assist with pre-operative care.
Monitor patients for complications, signs of infection, and organ rejection.
Administer medications, including immunosuppressive therapy, as prescribed.
Coordinate with surgeons, physicians, coordinators, and other healthcare professionals.
Maintain accurate patient records and documentation.
Ensure compliance with hospital policies, transplant protocols, and infection control practices.
Arrange follow-up appointments and provide discharge education.
Offer emotional support and counseling to patients and their families.
Participate in quality improvement and transplant program activities.
