- Roles & Responsilbilities:
- Assessment:
- Conduct home or site visits to evaluate women`s' living situations.
- Identify physical, emotional, and social needs.
- Support and Advocacy:
- Advocate for clients' rights and access to services.
- Help clients navigate social services, healthcare, housing, and legal systems.
- Case Management:
- Develop and implement service plans.
- Monitor client progress and update care plans as needed.
- Coordinate services across agencies (e.g., schools, clinics, legal aid).
- Crisis Intervention:
- Respond to emergency situations such as domestic violence, child neglect, or homelessness.
- Provide immediate support and refer to appropriate services.
- Documentation and Reporting:
- Maintain detailed records of client interactions and services provided.
- Prepare reports for supervisors, courts, or funding agencies.
- Community Outreach:
- Build relationships with local organizations and stakeholders.
- Educate the public about available resources and support programs.
- Counseling and Emotional Support:
- Offer counseling or refer clients to mental health professionals.
- Support clients in developing coping strategies and decision-making skills.
- Monitoring and Evaluation:
- Track outcomes to assess the effectiveness of interventions.
- Adjust strategies based on feedback and changing needs.
