Role and Responsibility :- i) Identification of new PLHIV and Registering them for CSC. ii) Conduct Field Visit and participates in the awareness generation activities . iii) Conduct Home visit for the registered PLHIV and affected families to provide treatment education, OIS, Nutrition , Healthy Lifestyles and positive Living . iv) Track LFU and MIS Client and Track back to ART Center and report to your coordinator. v) Facilitate Linkage of ICTC identified reactive Clients to the nearest ART center ,ensuring prompt treatment Initiation. vi) Develop weekly and monthly work plan , including Details of activities such as home visit LFU follow up Advocacy events , Visit to health facilities , meeting in CSC , and participation in support group meeting.
