Reports To: Senior Consultant / Medical Superintendent
Shift Timing: 10:00 PM to 10:00 AM (Night Duty — 12 Hours); rotational shifts may also be required

About the Role
We are looking for a skilled and confident Duty Doctor to manage critical care and emergency patients during the night shift (10 PM – 10 AM). You will be the primary physician responsible for initial assessment, stabilisation, and management of critically ill and emergency patients across the CCU and Casualty Department, working closely with nursing staff and escalating to the on-call consultant when required. This role is best suited for a doctor with hands-on experience in critical care and emergency medicine environments, capable of maintaining composure and making sound clinical decisions in high-pressure situations.
Key Responsibilities
Critical Care Unit (CCU)
- Monitor and manage CCU patients during the shift including hemodynamics, ventilator parameters, fluid balance, and sedation levels
- Monitor mechanical ventilation and assist in ventilator adjustments across common modes (AC, PC, SIMV)
- Initiate and monitor vasopressor and inotropic support such as Noradrenaline, Dopamine, and Dobutamine as per protocol
- Perform and interpret arterial blood gas (ABG) analysis and guide fluid resuscitation and electrolyte correction
- Respond to cardiac arrests, arrhythmias, and hemodynamic deterioration and initiate emergency management protocols
- Implement ICU protocols including sepsis bundles, DVT prophylaxis, stress ulcer prophylaxis, and nutrition monitoring
- Interpret ECGs, chest X-rays, and critical laboratory values and escalate abnormal findings when required
- Maintain accurate clinical documentation including progress notes, medication orders, and treatment instructions
Casualty / Emergency Department
- Triage and assess patients presenting to the Casualty / Emergency Department during the night shift
- Initiate management of acute medical emergencies including myocardial infarction, stroke, DKA, HHS, COPD exacerbation, pulmonary oedema, anaphylaxis, and polytrauma
- Order and interpret emergency investigations including CBC, metabolic panels, troponin, coagulation tests, and point-of-care diagnostics
- Perform or assist with basic emergency procedures such as IV access, NG tube insertion, urinary catheterisation, wound suturing, and fracture immobilisation
- Coordinate specialist consultations, admissions, and inter-department patient transfers
- Initiate thrombolysis or escalate for primary PCI in STEMI cases as per hospital protocol
- Manage psychiatric emergencies, poisoning, overdose, and substance intoxication in coordination with relevant specialists
- Document all Medico-Legal Cases (MLCs) accurately and notify authorities as per statutory guidelines
Required Skills & (Preferred Clinical Experience)
Candidates should have preferred clinical experience and working familiarity with the following areas.
Critical Care
- Mechanical ventilator monitoring and understanding of common ventilator modes (AC, PC, SIMV)
- Vasopressor and inotrope usage in ICU settings (Noradrenaline, Dopamine, Dobutamine)
- Arterial blood gas (ABG) interpretation and electrolyte management
- Continuous cardiac and hemodynamic monitoring
- Implementation of ICU protocols including sepsis bundles and critical care management pathways
Emergency / Casualty
- Emergency airway management and assisting with endotracheal intubation
- ACLS protocols and cardiac arrest management
- Trauma resuscitation principles (ABCDE / ATLS approach)
- ECG interpretation and recognition of acute coronary syndromes
- Emergency drug administration for anaphylaxis, shock, and seizures
- Initial management of medical emergencies such as myocardial infarction, stroke, DKA, HHS, COPD exacerbation, pulmonary oedema, poisoning, and overdose
Emergency Procedures
Candidates should be comfortable performing or assisting with the following procedures depending on training and credentialing:
- Endotracheal intubation
- Central venous catheter insertion
- Arterial line placement and ABG sampling
- IV access and emergency medication administration
- Wound closure and suturing
- Urinary catheterisation and NG tube insertion
- Defibrillation and cardioversion
Good to Have
- Point-of-Care Ultrasound (POCUS) for cardiac, lung, or FAST examination
- BLS, ACLS, or ATLS certifications (current or within the last 2 years)
- Experience using hospital EMR / HIS systems
- Familiarity with APACHE II, SOFA, GCS, and qSOFA scoring systems
Qualifications
- MBBS from a recognised university — Mandatory
- Minimum 2 years post-MBBS hands-on experience in CCU and/or Casualty — Mandatory for Opening 1
- Minimum 5 years post-MBBS hands-on experience in CCU and/or Casualty — Mandatory for Opening 2
- Valid MCI / NMC / State Medical Council registration — Mandatory
Salary
- 2 Years Experience — ₹40,000 per month
- 5 Years Experience — ₹50,000 per month
