Job title (Deputy Manager /Manager) (Max Saket)

ROLES AND RESPONSIBILITIES:
Clinical Oversight and Coordination

Conduct daily clinical rounds for all elderly patients tagged under the program.

Act as the primary liaison with the assigned MANCO member to coordinate timely interventions.

Monitor and ensure appropriate clinical prioritization of diagnostics, procedures, and consults for elderly patients.
Nursing Leadership and Supervision

Ensure only A & B category nurses are deployed to elderly patients; assess competency and training needs.

Provide mentoring, support, and supervision to bedside nurses caring for elderly patients.

Ensure nursing care plans are geriatric-specific and focused on functional preservation, comfort, and dignity.
Process Facilitation and Operational Support

Facilitate in-room admissions and discharges, minimizing patient movement and discomfort.

Ensure completion of admission assessments, discharge readiness checklists, and caregiver education.

Coordinate with housekeeping, nutrition, pharmacy, physiotherapy, and diagnostics to streamline elderly-friendly services.
Data Monitoring & Reporting

Maintain real-time tracking of all patients aged 75+ across the hospital.

Analyze clinical and operational data to identify trends, delays, or gaps in care.

Submit weekly reports to leadership highlighting patient outcomes, incident reports (if any), and improvement opportunities.
Patient & Family Engagement

Ensure regular communication with patients and families regarding care plans, expectations, and discharge planning.

Escalate psychosocial or support needs, as appropriate.
Quality, Safety & Experience

Monitor adherence to geriatric safety protocols (falls, pressure injuries, medications).

Ensure zero delay in care.
Support nursing audits, patient feedback mechanisms, and implementation of improvement projects.
Experience & Qualifications
-
8 to 10 years of relevant experience

Healthcare is preferable
