Full job description

We are looking for an experienced and detail-oriented Medical Records / EHR Documentation Specialist to support our healthcare operations.

The ideal candidate must have hands-on experience working with patient charts in an EHR/EMR system, including accurately entering or adding progress notes, reviewing documentation, and properly signing and locking completed notes/charts according to the provider's established workflow and protocol.

This position requires a very high level of accuracy, attention to detail, confidentiality, and understanding of healthcare documentation processes.

Key Responsibilities

Add and enter progress notes into patient charts accurately.
Follow the provider's established documentation protocol and workflow.
Review entered information carefully for completeness and accuracy.
Properly sign and lock progress notes/charts when authorized and instructed.
Ensure documentation is entered under the correct patient and encounter.
Maintain accurate and organized electronic medical records.
Follow all provider-specific instructions regarding documentation.
Identify discrepancies or missing information and escalate them appropriately.
Maintain strict confidentiality of patient information.
Follow HIPAA and applicable healthcare privacy and security requirements.
Perform quality checks before finalizing documentation.
Communicate with the provider or internal team when clarification is required.

Required Qualifications

Previous experience working with EHR/EMR systems.
Direct experience entering progress notes or clinical documentation into patient charts.
Experience with signing and locking notes/charts.
Strong attention to detail and accuracy.
Ability to follow established protocols exactly.
Excellent written and verbal communication skills.
Ability to work with confidential patient information.
Understanding of HIPAA and patient-data confidentiality.
Ability to work independently and meet deadlines.

Preferred Qualifications

Experience working with a cardiology practice or other specialty medical practice.
Experience in medical billing, medical records, clinical documentation, or RCM.
Experience with platforms such as Tebra, athenahealth, eClinicalWorks, Epic, NextGen, AdvancedMD, Kareo, or similar EHR/EMR systems.
Previous experience supporting U.S. healthcare providers.
Experience working remotely with U.S. healthcare organizations.

Important Requirement

The selected candidate will be expected to demonstrate their understanding of the workflow and follow the exact documentation protocol provided by the physician/practice.

Because this position involves sensitive patient information, accuracy and confidentiality are extremely important.
