Support Coordinator

Primacy In-Home Care (In Partnership with Primacy Care Australia)
Location: All Kerala
Job Type: Full-time

About the Role

Primacy In Home Care is seeking a dedicated and compassionate Support Coordinator to coordinate home care services, support clients and caregivers and ensure high-quality care delivery. The ideal candidate should possess strong communication, coordination and problem-solving skills with a passion for serving older adults and individuals requiring home-based care.

Qualifications

Master's Degree in Social Work (MSW) – Mandatory
Fresh graduates may apply
Candidates with experience in home care, healthcare, elder care or community services will be given preference
Excellent communication and interpersonal skills
Ability to coordinate with clients, families, caregivers and healthcare professionals
Good documentation and reporting skills
Proficiency in MS Office and digital communication tools

Key Responsibilities

Coordinate and monitor home care service delivery
Conduct client follow-ups and maintain service records
Liaise with clients, families and healthcare professionals
Conduct client assessment, Assist in client care plan and care coordination
Handle service inquiries and ensure client satisfaction
Prepare reports and maintain documentation
Support operational and quality assurance activities

Preferred Skills

Strong organizational and time-management abilities
Empathy and client-centered approach
Ability to work independently and as part of a team
Knowledge of geriatric care and home care services is an advantage
