A. PATIENT CARE
- Assess patients and identify immediate nursing-care requirements.
- Provide appropriate nursing care according to the patient's condition and doctor's orders.
- Monitor and document vital signs, pain, intake/output, neurological status, and other relevant parameters.
- Administer medications, IV fluids, injections, blood products, and other treatments as prescribed.
- Monitor patients for deterioration and immediately inform the treating doctor.
- Maintain accurate nursing notes, medication records, treatment charts, and patient documentation.
- Ensure proper patient identification before medications, procedures, investigations, and transfusions.
- Maintain patient privacy, dignity, comfort, and confidentiality.
3. EMERGENCY ROOM (ER) RESPONSIBILITIES
- Receive and promptly assess emergency patients according to hospital protocol.
- Assist doctors during emergency management and resuscitation.
- Maintain readiness of emergency equipment, crash cart, oxygen, suction, emergency medicines, and essential supplies.
- Assist in CPR/BLS and other emergency procedures within the nurse's scope of practice.
- Establish and maintain IV access as required and monitor IV therapy.
- Collect and send emergency samples for laboratory investigations.
- Assist with wound care, dressings, suturing, procedures, and emergency interventions.
- Coordinate investigations, admissions, referrals, and transfers.
- Maintain complete emergency documentation and handover.
4. WARD RESPONSIBILITIES
- Manage nursing care for admitted patients.
- Receive patients from ER, OT, ICU, or other departments with proper handover.
- Carry out doctor's orders accurately and promptly.
- Administer medications and treatments according to hospital policy.
- Monitor postoperative and medically ill patients and report changes immediately.
- Assist doctors during bedside procedures.
- Maintain medication charts, nursing notes, fluid charts, observation charts, and other records.
- Educate patients and attendants regarding medication, diet, hygiene, mobility, and discharge instructions.
- Coordinate patient investigations, procedures, consultations, and transfers.
- Ensure proper preparation of patients for surgery, procedures, and diagnostic investigations.
- Assist with discharge procedures and ensure completion of required nursing documentation.
5. ICU RESPONSIBILITIES
- Provide close monitoring and nursing care to critically ill patients.
- Monitor ECG, SpO₂, blood pressure, respiratory rate, temperature, urine output, and other parameters as required.
- Identify and promptly report any deterioration in the patient's condition.
- Administer critical medications, IV infusions, antibiotics, and other prescribed treatments safely.
- Manage and monitor oxygen therapy and assist with ventilator care according to training and hospital protocols.
- Assist doctors during intubation, central-line procedures, emergency procedures, and resuscitation.
- Maintain ICU equipment, monitors, infusion pumps, suction, oxygen, and emergency equipment in working condition.
- Maintain strict infection-control and aseptic practices.
- Maintain accurate ICU charts, medication records, fluid balance, nursing notes, and critical-care documentation.
- Ensure proper shift-to-shift handover of critically ill patients.
6. MEDICATION & TREATMENT SAFETY
- Follow the "right patient, right medication, right dose, right route, right time" principles.
- Check medication orders before administration.
- Verify allergies and relevant precautions.
- Monitor patients for adverse drug reactions and report immediately.
- Maintain proper medication storage and controlled-drug documentation as applicable.
- Never administer medication without a valid order except where covered by approved emergency protocols.
7. INFECTION CONTROL
- Strictly follow hospital infection-control policies.
- Follow hand hygiene and appropriate PPE practices.
- Maintain aseptic technique during procedures and invasive-care activities.
- Follow biomedical waste segregation and disposal procedures.
- Assist in preventing hospital-acquired infections.
- Maintain cleanliness and infection-control standards in ER, Ward, and ICU areas.
8. SUPERVISION OF JUNIOR STAFF
- Supervise junior nurses, nursing assistants, and support staff during assigned shifts.
- Allocate duties according to patient load and staff availability.
- Ensure timely completion of nursing duties and documentation.
- Guide junior staff in clinical procedures and hospital protocols.
- Monitor medication administration, patient monitoring, and basic nursing standards.
- Provide appropriate training and on-the-job guidance.
- Report staff performance issues, absenteeism, or misconduct to the Nursing In-charge.
9. EQUIPMENT & EMERGENCY READINESS
- Ensure essential nursing and emergency equipment is available and functional.
- Check crash cart and emergency supplies according to hospital schedule.
- Ensure oxygen cylinders/central oxygen supply, suction, Ambu bag, monitors, infusion pumps, and other essential equipment are ready.
- Immediately report equipment malfunction or shortage.
- Maintain appropriate records of equipment checks.
10. DOCTOR & DEPARTMENT COORDINATION
Coordinate effectively with:
- Consultants and Duty Doctors
- Emergency Department
- ICU
- Operation Theatre
- Laboratory
- Radiology
- Pharmacy
- Physiotherapy
- Reception / Billing
- Housekeeping
- Administration
Ensure timely communication of patient-related information and urgent clinical changes.
11. PATIENT & ATTENDANT COMMUNICATION
- Communicate respectfully and professionally with patients and attendants.
- Explain routine nursing procedures and care requirements.
- Respond appropriately to patient concerns.
- Escalate complaints and difficult situations to the Nursing In-charge/Management.
- Maintain confidentiality and do not disclose unauthorized medical information.
12. HANDOVER & DOCUMENTATION
- Conduct proper verbal and written shift handover.
- Clearly communicate critical patients, pending investigations, medication schedules, procedures, and important observations.
- Ensure all nursing documentation is complete, accurate, and up to date.
- Never alter or falsify patient records.
- Maintain confidentiality of all patient information.
13. QUALITY & PATIENT SAFETY
- Follow hospital SOPs, nursing protocols, and patient-safety standards.
- Ensure fall-prevention measures for at-risk patients.
- Follow pressure-injury prevention practices.
- Maintain proper identification and allergy-alert procedures.
- Report medication errors, patient falls, needle-stick injuries, adverse events, and other incidents immediately.
- Participate in nursing audits, training, and quality-improvement activities.
