Clinical Documentation & EMR Assistant / Medical Transcriptionist for Reputed Urology Clinic in South Kolkata

Job Type: Full-time, On-site

Salary: ₹22,000–₹28,000 per month, depending on experience and skills

Job Summary

A reputed Urology Clinic in South Kolkata is looking for a smart, organised and computer-proficient Clinical Documentation & EMR Assistant / Medical Transcriptionist to support a busy specialist Urology practice.

The candidate will work closely with the Consultant Urologist and assist with pre-consultation documentation, organisation of medical records, EMR data entry and AI/voice-assisted clinical documentation.

We are looking for someone who can do more than routine typing or transcription. The ideal candidate should be able to understand medical terminology, quickly organise information from multiple medical reports and prepare an accurate, concise patient record for the doctor.

Key Responsibilities

Obtain and document basic patient history before consultation.
Record presenting complaints, duration, relevant medical/surgical history, current medications and allergies.
Organise previous prescriptions, discharge summaries and medical records chronologically.
Identify and accurately enter relevant laboratory investigation results into the EMR.
Organise and upload USG, CT, MRI, PET-CT, pathology and other investigation reports.
Prepare a concise pre-consultation patient summary for the Consultant.
Assist the Consultant with real-time clinical documentation during OPD.
Enter findings and instructions dictated by the Consultant.
Use voice-to-text and AI-assisted documentation tools.
Review AI-generated documentation for transcription or factual errors before Consultant verification.
Maintain accurate and well-organised electronic patient records.
Assist with documentation for investigations, procedures, admissions and follow-up.
Maintain strict confidentiality of all patient information.

Candidate Requirements

Essential:

Good knowledge of medical terminology.
Excellent written English and comprehension.
Good typing speed with high accuracy.
Strong computer skills.
Ability to understand and extract relevant information from medical reports.
Excellent attention to detail.
Ability to learn new software quickly.
Professional and courteous communication with patients.
Willingness to work full-time, on-site in Kolkata.

Preferred:

2+ years’ experience in Medical Transcription, Medical Scribing, Healthcare Documentation, EMR/EHR or Medical Records.
Experience working with hospitals, clinics, diagnostic centres or healthcare BPOs.
Familiarity with laboratory, radiology and pathology reports.
Previous exposure to surgical/Urology terminology.
Experience with voice recognition, AI documentation or medical scribe software.

Important Role Boundary

This is a clinical documentation role and not an independent medical practitioner position.

The employee will record and organise factual medical information. Diagnosis, interpretation of investigations, prescriptions and treatment decisions will remain the responsibility of the treating doctor.

Compensation

Initial probation period: 3 months.
